Can I view my mental health records?

What are my rights in Portland, Oregon for viewing my mental health record and doctors notes? I'm in the process of changing practioners and I have to sign a medical release at the new clinic. I had a HORRIBLE!!! expericence at my current office with my nurse practioner. I don't want my new provider to recieve incorrect information from my records.
